How to Schedule
Please call 708-801-1005 or email precisionptspecialists@gmail.com. Phone calls will be returned within 24 hours. If you would like us to reach out to you, please click the Appointment Request link below.
We will encourage you to schedule 3-4 weeks of appointments with your initial evaluation to ensure continuity of care. You are not obligated to schedule beyond the initial evaluation or to keep the additional appointments if they are not necessary based on your plan of care.
Precision Physical Therapy Specialists, PLLC uses the digital patient intake program from WebPT. If you would prefer to fill out a paper intake form, it can be found here. Our HIPAA and Consent for Care and Treatment forms can be found here.
